    The Health System of the Future: Transforming Post-Acute Care

    Dr. Josh Luke, CEO, Futurist, Keynote Speaker

    More and more patients are being discharged to home after a hospitalization. A change in payment models is part of this shift to home-based post-acute care. In this session, Dr. Luke will explain the “Discharge with Dignity” case management tool, he designed, which is a discharge methodology prioritizing home-based discharges that has been implemented by health systems throughout the country. Dr. Luke will also describe new technologies allowing quick, safe transfers that enhance data capabilities. When a discharge to home is not possible, Dr. Luke will discuss how hospitals and post-acute providers can work together to facilitate discharges of complex hospital patients to an appropriate care setting.

    Developing Partnerships to Bridge Transitions of Care

    Judy Baskins, Former Chief, Clinical Integration and Ambulatory Services, Palmetto Health

    The inaccessibility of post-acute care for many hospital patients with complex needs results in those patients staying in the hospital much longer than necessary or appropriate. In this session, Ms. Baskins will relate her experience as Chief of Clinical Integration and Ambulatory Services for Palmetto Health in Columbia, South Carolina.  In that role, Ms. Baskins oversaw home health, hospice, private duty services, outpatient rehabilitation services, skilled nursing facility, case management and discharge planning, care coordination and transitions of care efforts and the development and implementation of post-acute care services. Ms. Baskins will describe how she convened a group of health system, post-acute and Medicaid representatives to devise a sub-acute funding mechanism to care for complex patients after discharge.
