Patient Activation

A powerful predictor of health care utilisation that clinicians can use to enhance the effectiveness and ease of their consultations with patients with long-term conditions.

  • Understand the scientific basis for how patient activation scores predict patient health and health resources utilization, including Emergency Department visits.
  • Learn how clinicians, informed about individual patient's activation level, tailor their consultations to 'walk along side' each patient, accelerating their journey to higher levels of activation.
  • Experience video enactment of patient encounters that bring to life the differences in activation levels
  • Observe the difference between typical consultations with patients with long term conditions compared to patient activation informed-consultations to experience the ease and efficiency for clinicians when they use techniques to 'walk along side' the patient.
