8th Annual Palliative Care Conference

Let us learn from each other as we share best practices in palliative and end-of-life care.  The conference will highlight practical methods and tools to support our patients and families achieve their best possible outcomes at the end-of-life.  

As part of the conference, you, and your colleagues have the privilege of learning from Bud Hammes of Gundersen Health System in Wisconsin.  Gundersen Health, through its Respecting Choices efforts, has engaged its whole community in advance care planning.  Dr. Hammes will deliver the keynote plenary about their work designing a system that supports clinicians and engages patients in advance care planning.  In addition,  we have a wonderful slate of speakers who will share their expertise in a variety of areas ranging from system design,  analyzing and using big data, and forging ways to provide palliative care in the community.   We hope you will join us.
