Transforming Transitions of Care: Cultivating teamwork

and enhancing communication

Program Overview

The purpose of this summit is to bring together health care professionals to discuss transitions of care across various settings and levels of care. We will explore barriers to coordination and discuss the role of teams during transitions. The morning session will focus on the teaming aspect of care coordination and in the afternoon session we will turn our attention to a particular transition, palliative care.

Learning Objectives:

1.  Describe the importance of interdisciplinary collaboration in planning care for patients who are transitioning between levels of care or along the health continuum.

2.  Identify challenges during transfers of care and utilize evidence-based strategies to improve patient satisfaction and outcomes, and diminish fragmentation of care.

3.  Effectively facilitate compassionate patient-centered care for patients experiencing life-altering medical conditions and identify resources that support coordination of care for transitional populations.
