Care Transitions Pathways (October 25, 2017)

 

Overview


The Care Transitions Pathway is an integral program to ensuring patients transition from an inpatient setting to the home environment in an efficient manner.  As a Care Transitions Coordinator (CTC) you will ensure continuity of patient care through the coordination among all health professionals involved in transitioning the patient home SAFELY. It is designed to ensure all patient care needs are met for a successful outcome.  The CTC integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care.  The CTC promotes adherence to post-acute plans and ensures ordered services are completed. The CTC represents the Agency in transitional care activities, strategic relationships with health systems, hospitals and other inpatient facilities, physicians and physician groups, and executive level opportunities.  Join us to learn valuable techniques and information that will help you maximize your CTC position and network with others in the CTC role.

Duration


2 days of training; Course concludes at 3pm on the last day


Materials to Bring


CTCs are to bring their manuals with them.  If they do not have one, it will be provided at the course.
