BO1 - 02: Small Clinic Empowerment: Data Independence through Clinical Interfaces

Two CHUGs ago we presented how we use a business plan in our small clinic to adopt and implement better technologies and workflows in a meaningful way. This past fall we presented how we leveraged our technologies to achieve meaningful use. Much of the interest following the presentation focused on our use of interfaces and web-enabled technologies to leverage our data in a meaningful way. We would like to present a more in-depth look at our use of the clinic interface engine, portal, lab results interface and secure messaging. One example is our single lab ordering interface that provides enhanced workflow at the point of care reducing overhead, transmits billing information automatically into CPS 10, brings a patient friendly, editable graphic result to the physician's desktop that can be sent to the patient via secure messaging. Our interface engine allows us to participate in current and future registries as well as participation in future CCD document upload and download. The coordination of these interfaces with our existing portal and secure messaging capabilities enable us to be confident we will be able to participate in the rapidly evolving healthcare market without sacrificing the advantages of our small, privately owned and controlled clinic. Objectives: - Have a good understanding for the interfaces that are available for small clinic usage in a CPS 10 environment, and how they are seen in the light of a profitable business plan.

BO1 - 03: Techniques for EMR Harmony: Implementation and Enhancement with Minimum Revolt

In 2007, the University of Oklahoma Family Medicine clinic transitioned our paper records to Centricity EMR as part of a larger implementation within a 500-physician academic practice group. In anticipation of resistance to implementation, especially from more established physicians, we developed an innovative strategy for orientation and skills development within the Department of Family and Preventive Medicine's 50 physician practice group. The innovation involved the creation of a provider/staff education team comprised of the departmental IT director and a physician champion, and includes monthly EMR meetings at which faculty have the opportunity to participate in troubleshooting and solution sharing with campus EMR management, and a quarterly "EMR Tips and Tricks/EMR Best Practices" presentation at departmental Grand Rounds. Our response to implementation has been excellent, with faculty, staff and residents expressing increased satisfaction with and comfort in use of the Centricity EMR. In addition, by using as a physician champion a faculty member who also teaches billing, coding and risk management and who is the departmental faculty representative to the Meaningful Use and Patient-Centered Medical Home campus committees, we have ensured the continued interest of the faculty and residents in improving their skills within the EMR. We will discuss what we as a department have learned in the implementation and, perhaps more importantly, in maintaining ongoing communication with a disparate group of Physician faculty, Physician Assistants, students, residents and staff to enhance use of the EMR to meet the primary goal of excellence in patient care. Objectives: - Understand the correlation between efficient, effective patient care and the knowledge and comfort level of physicians and staff with the technological tools provided for their use at the point of care.
- Be able to identify barriers against and motivators for physicians to spend the time and energy necessary to become high functioning users of an electronic medical record system.

BO2 - 04: Triple Aim - How We Got There

In 2008, Sound Family Medicine (SFM), a 32 provider primary care group located in Puyallup, WA received a grant from the Washington Health Information Collaborative to create a disease management registry (DMR). Having implemented the Centricity EMR in 1998, SFM already had a rich repository of clinical information about their patient population. Their goal was to utilize this data to identify chronic disease patients in need of care. In doing so, they hoped to increase the awareness of both these patients and their providers and take a proactive approach to chronic disease management. SFM was able to do just that. The resulting system has allowed SFM to combine diagnosis, observation, encounter, and other clinical information to systematically recall high risk patient populations. This system has also allowed them to generate and deliver targeted communications to these patients regarding their disease status and need for care. This has resulted in increased patient visits, improved quality, better disease management and reporting, and increased revenue. Overall, SFM has seen a dramatic increase in patient compliance as reflected in their diabetic and preventative care measures (Measured using NCQA and DOQ-IT Preventative Medicine measures generated by MQIC). Objectives: - Understand how to utilize data that's already there in Centricity to improve patient outcomes.

The panel will describe the uses and value of the de-identification process from regulatory, statistical, public-policy, and legal points of view and will discuss the nuances of appropriate use of EMR data for medical research. EMR data contain a wealth of information that can enhance medical knowledge through research. While the need to protect patients’ privacy is paramount, there is also a need to make research data available for the benefit of the whole community. To meet both of these needs, the HIPAA Privacy Rule provides a framework for removing data from protected health information to prevent the individual patient from being identified. Once data are de-identified in this manner, they are no longer considered protected and may be used for research or disclosed for other purposes.

Patient education handouts can be a valuable tool in improving quality of care by increasing patient compliance and provider-patient communication. Centricity’s patient education functionality is powerful when utilized to its full potential. In this presentation we will explore how our pediatric practice has created custom handouts and letters over the last several years and how they will be used to meet Meaningful Use benchmarks. We will explore the layout of our VFE “Handout Dashboard” form. This form streamlines the process of finding the right documents with fewer clicks and without custom handout lists. Reportable documentation that handouts were giving can be achieved without creating a new visit using that form, as will be demonstrated with in depth examination of the functionality of those buttons and associations. We will introduce the ways in which we use observation terms, document variables and patient attributes to create handouts that are visit-specific without manual editing. Health Coaches at Blue Hill Memorial Hospital (Maine) are skilled nurses (RN) who take a leadership role in the team overseeing patient care. Our Health Coaches use a specific visit workflow that integrates the Centricity EMR with guideline-based, motivational education tool ScoreHealth. This workflow combines two strong technologies to implement outcome based care and population management. Trends in the healthcare industry are now moving towards a more outcome-based approach. BO8 - 03: Implementation of Advanced Auto-Indexing at a Federally Qualified Health Center
Roger Cortez, Director of Health Information Management, Lawndale Christian Health Center Bruce Rowell, Medical Director of Clinical Quality, Lawndale Christian Health Center Lawndale Christian Health Center (LCHC) is a multi-specialty Federally Qualified Health Center (FQHC) in the west side of Chicago. We serve over 40,000 patients with 140,000+ visits per year with 70 providers at four locations. Centricity EMR went live in 2008. By 2011, we were importing about 12,000 documents per month and so began evaluating solutions to reduce this burden on resources and improve our efficiency. Any solution would also need to improve the accuracy of our data collection, something critical to our reporting requirements. Leadership at LCHC selected InDxLogic (IDL) as our vendor and gathered the internal resources to make sure the project was successful. In the first six months, the IDL tool automated the indexing of 60% of clinical documents. The automated process now allows us to import documents and capture clean, accurate clinical data in a cost-effective and timely manner. This presentation describes the team-driven approach to implementing IDL and the internal operational and workflow changes required for AAI to have the highest match rate possible, as well as the tools developed to track efficiency and accuracy of the data being imported.
