The importance of failure investigation is emphasised in many equipment integrity management practices and promoted by the HSE and part of demonstrating responsible operating management. Understanding why equipment fails is a key requirement of good safety management and plant reliability improvement.

If an investigation is carried out well, it is a very powerful tool to prevent future equipment failure of deterioration. If it is not done effectively, then it may result in considerable time and expense, with no meaningful conclusions. The objective of this talk is to show how the investigation process can be carried out, to achieve the greatest chance of success and identification of actions to prevent a recurrence.
