This form is used to register for the event. You may book and pay for one or more attendee on this form. Please note that registrations close 2 days before the date of the event. The fee includes all coffee breaks, a luncheon and seminar materials.

Group Bookings
If you are registering for someone else, enter the registrant's details and their email address below and a confirmation and tax invoice will be sent to their email address. On the registration page you have the option of entering your email address in the "CC Email for Tax Invoice" so that you will also receive a copy of the email confirmation and tax invoice.

Payment - via Credit Card Only
Payment must be made by Credit Card (Visa, MasterCard, or American Express) at the time of booking. Credit card payments are processed via our secure registration provider and your credit card statement will show a debit to "ACUVENT" for the registration fee.

Changes & Cancellations
Only cancellations made at least 7 working days prior to the seminar will receive a refund. A cancellation fee of $70 applies. Should you be unable to attend, a substitute delegate is welcome at no extra charge.

Once you have registered, you will be sent a confirmation email with a link to enable you to login and make any changes or cancellations to your registration.
