Health Care Planning for Life (new!)

Training Description

Few people want or like to think about the negative "what ifs" in life, but on too many occasions family’s lives are caught off-guard when something happens at any age or stage without the proper paperwork in place. This class is an opportunity to learn how to put documents in place for you and your residents that will inform the medical community and loved ones (and remove the burden from them) by creating a Health Care Plan. Attendees will learn what a health care plan is, what forms and documents are involved (e.g. Health Care Proxy, Living Will, Durable Power of Attorney, MOLST, CC/DNR), which forms are legal, and which are for informational purposes, and with whom to communicate their plans. Everyone 18 years and older has the right to be fully informed about their health care and make choices based on their goals, values, and beliefs. It's your life, it's your choice!
