Online via Credit Card Payment (preferred method)
Complete the on-line registration process – click on the Register button on the bottom of this page. Credit card payment must accompany your registration. Please complete a separate registration form for each person attending.

Online via Check Payment
Complete the on-line registration process (click on the Register button on the bottom of this page), and select the by check payment method when you reach the payment screen. Use the registration confirmation email as your invoice, and mail your check (payable to IDMA) to the below address:

Confirmations, Receipts and Invoices
If you register online with a credit card, you will receive a final confirmation e-mail immediately after submitting your registration. Please keep your confirmation — it is also your receipt.

If you choose to pay by check, we will send you an “initial” registration confirmation email, which will also serve as your invoice. Once we receive your check, we will provide you with a final registration confirmation, which will include your payment receipt.

Registration Policy
The registration fee includes two receptions, two lunches, two continental breakfasts, and four refreshment breaks, as well as all conference materials. Travel and hotel accommodation costs are not included and are the responsibility of each conference participant. Unless you are paying by check, payment must accompany your registration form.

If you are attending as a substitute for someone who's already registered, you must complete a registration form, indicating the name of the person you are replacing.

Cancellation Policy and IDMA’s Liability
All cancellations and refund requests must be sent to the IDMA office via email no later than March 17, 2017 and will be subject to a $235 administrative fee. Please note that cancellations received after March 17, 2017 will be liable for the full seminar fee. Substitutions can be made at any time. Refer to the registration policies for arranging substitutes. If, for any reason, the event is canceled, IDMA will refund the total registration fee. However, IDMA will not be responsible for any travel, hotel accommodations, or other costs incurred.
