Improving the efficiency of retrospective hazard reviews, the challenges of cost and resource - Lunchtime Forum

Many companies face a requirement to retrospectively review their hazardous process facilities, to meet company standards or Regulator led duties to demonstrate continuous improvement in process safety. 

This forum will reflect on ABB's practical experience gained in carrying out these reviews efficiently and dealing with subsequent recommendations. Process safety reviews on operating facilities provide unique challenges compared to project stage hazard studies, such as poor 
quality P & IDs and difficulty in gaining the time commitment of busy operations staff. 

Some key learning points from applying study techniques retrospectively, including the need for high quality records and typical recommendations found on existing facilities will be discussed. Following the team based reviews there is still much to be done, and guidance will be given on effective close-out of actions, keeping the records 'evergreen', and the choice of approach for subsequent revalidation, typically every 5 years.
