Medical Rehabilitation Webinar: One Year Post Severe Rehabilitation Outcomes and Utilization of Follow-Up Services: Identifying Those in Need

Summary

Although the CARF standards in rehabilitation require that patient outcomes following discharge be assessed and opportunities for follow-up made available, many programs have found implementing this in a cost-effective manner to be challenging. Madonna Rehabilitation Hospital in Lincoln, Nebraska, developed an interviewing tool with subsets of questions customized for brain injury, spinal cord injury, stroke, pediatrics, amputation, and neurological conditions that allows for the collection of key outcomes data while also screening, at the same time, for the need for follow-up services. This model benefits society by providing services only to those who need them, and, in the age of healthcare reform, this is likely to be an increasing expectation. This model also benefits the facility by reducing the no-show rate and freeing up clinicians for services that are necessary, where they feel they can contribute significantly to quality of life. Perhaps most importantly, it benefits the client by identifying those in need who want services, therefore ensuring personal choice. The information gathered includes impairments, activities, and participation and will be summarized by diagnostic group. Descriptions of how this tool guides further care will be provided.

Webinar participants should be able to:

  • Describe a cost-effective model for collecting long-term outcomes data and identifying those in need of follow-up services one year following receiving rehabilitation services.
  • Describe long-term outcomes data regarding impairments, activities, and participation for a population of patients one year post rehabilitation.
  • Describe ways to effectively utilize the data collected, including program development and referral/payer relations.
