Memory Care Mentor (March 14-15, 2018)

 

Overview

This Memory Care Mentor course is for Encompass clinicians who have a desire to mentor their colleagues in caring for patients with cognitive impairment. This could be considered “Memory Care 2.0.” Patients with cognitive impairment are clinically challenging, as is applying evidence-based practices that are effective in achieving the goals of program. It is important that Encompass Memory Care Mentors document defensibly using the Memory Care Pathways, and that they be able to articulate the tenets and goals of the program, as well mentoring local colleagues in developing and carrying out a plan of care for the cognitively impaired patient.

Objectives of the training:

1. Develop local Memory Care “experts” whose practice and documentation
    show consistency with the tenets of the Encompass Memory Care program.


2. Roles for local MC Mentors:
    a. Locally precept MC centralized training (presented by webinar from
        Home Office).
    b. Do community presentations on Memory Care using the Memory Care
        educational flyers  and Speakers Notes. The local mentor would work
        with the AM on scheduling these.

    c. Provide follow-up brief presentations during case conference to focus
        on specific clinical areas (such as, how to prevent hospitalization in
        the MC population).

    d. Assist in auditing charts using approved MC audit tool.

Requirements:

1. Priority will be given to the following:
    a. EDC Preceptors in good standing.
    b. Clinicians with 90% or more on Field Staff Quality Ranking Report in
        Tableau.

    c. OTs and SLPs.

2. Required prerequisites:
    a. Participation in Memory Care chart audit tool training. (1 productivity
        point).

    b. Audit of 10 charts (5 of own).
    c. Participation in live Memory Care training or viewed all 3 Memory Care
        modules within last year.
    d. Must be willing to assist with local Memory Care presentations and
        community presentations utilizing MC flyers and approved speakers
        notes.

OTs, SLPs, RNs and PTs who have already been approved to attend

2 days of training; Course concludes at 3pm on the last day
