The National Association of Free and Charitable Clinics (NAFC) will be holding a one day free medical clinic on Wednesday, July 3, 2013 in New Orleans, Louisiana at the Ernest N. Morial Convention Center for those in the area who have no or limited access to health care.
The NOLA C.A.R.E. (Communities Are Responding Everyday) Clinic will offer free basic medical care and health education to the area's medically underserved. Being a volunteer at this event will give you the opportunity to help hundreds of people gain access to medical care they otherwise may not receive.

OUT OF STATE PRACTITIONER VOLUNTEERS - Please see the documents below that you need to fill out and return to the Louisiana State Board of Medical Examiners either via email or fax to apply for a temporary license. Please let us know when you have submitted the information to the Board so we may also send a letter from the NAFC supporting your request. Additionally, please fax a copy of your license to 866-875-3827 or you may scan and email it to CARE@nafcclinics.org.

OUT OF STATE NURSES - To apply for a temporary license to be able to participate in this event, please download a copy of the attached form, fill it out and fax the completed form, your license, and drivers license to 225-755-7581. Please also fax your license to 866-875-3827 or you may scan and email to firstname.lastname@example.org.
Please note that this form may refer to Disasters, but will be utilized for this CARE Clinic volunteer event. Also please note that the NAFC must fill out the second page of the form to verify your participation, so please be sure to email it to CARE@nafcclinics.org and include "Out of State RN Application" in the subject line. Once both forms are completed and you gather the rest of the required documentation you can send to the LSBN.

Patients may call 800-340-1301 to schedule an appointment for the clinic. Patients are encouraged to call ahead and make an appointment for this clinic event. Walk-ins will be seen on a limited, first-come first-served basis.
- This event is for those WITHOUT insurance or with limited access to health care. Recipients of Medicare, Medicaid, or Veterans Administration benefits are not eligible.
- If you are taking medications, please bring your medicine bottles.
