This workshop will help participants improve the quality of their root cause analyses and action plans by introducing the National Patient Safety Foundation’s, RCA2: Improving Root Cause Analyses and Actions to Prevent Harm, and describing how it was implemented at Ochsner Health System, Louisiana’s largest non-profit, academic health care system. Ochsner Health System’s Director of System Quality & Patient Safety, Jessica Behrhorst, will facilitate the workshop, sharing strategies and lessons learned in their transition from no formal safety culture to full implementation of RCA2. The workshop will provide tools, resources, exercises for application, and the opportunity to ask questions and learn from someone with firsthand experience.The program will be free, and lunch provided.
