Patient Treatment:To bring and perform injections on your own patients, prior to the course you must complete a Participation Confirmation Agreement (including your active state license, National Provider Identifier (NPI), insurance provider contact and policy number), provide your insurance declaration page and a rider stating your malpractice insurance covers you in Nevada. Instructions for purchasing and transporting your injectables (not included in tuition fee) will be provided upon registration.
Prerequisites: No prior training with injectables is required.
