NOTE: If you currently serve on the DMI, NDB, or UDIA board of directors, please register through the email sent by Maribeth O'Leary on August 8th, or contact her directly at: email@example.com or fax 847-803-0183. Joint Annual Meeting Registration
Must be completed by Friday, September 23rd, for Midwest Dairy to issue a check for your $905 registration fee, or, if not received by that date, you will need to pay the registration fee on your own, send directly to DMI by Friday, October 7th and get reimbursed at a later date from Midwest Dairy. All registrations need to be paid prior to October 7th to avoid a $150 guest registration late fee.

NOTE: Midwest Dairy has set up a direct bill at the hotel and all hotel room reservations for UDIA delegates and alternates will be made by Midwest Dairy. Please include your hotel requests with your reservation. YOU DO NOT NEED TO MAKE YOUR OWN HOTEL ARRANGEMENTS.
Book your flights for the meeting directly with the airline or through a travel agent of your choice. If you wish to use Travel Leaders, you may call them at 1-800-686-9903 and work directly with JoAnn Beuch. If you book through Travel Leaders, Midwest Dairy will be billed directly for your airfare.
Personnel Audit Meeting/Corporate Board Meeting/Dinner
When making your travel arrangements, please be aware there will be a Midwest Dairy Corporate Board meeting on Monday, October 31st from 11:30 -1:30 p.m.
Personnel Audit Committee will meet on Monday, October 31st from 7:30 a.m. to 11:00 a.m.
Options for dinner will also be scheduled to take place following the UDIA House of Delegates meeting on October 31st.
Midwest Dairy’s reimbursement policy for this meeting is as follows (taken from p. 7 of the Directors Policy Manual): “3 per diem days for the 3-day event, plus payment for travel time not to exceed an additional per diem day.” Also, if you plan to arrive early or extend your stay, please refer to the enclosed Common Reimbursement Practices, and submit the Travel Costs Worksheet with your board expense voucher.
