Please read the following agreements. Indicate agreement below.
Agreement 1: Student agrees to the following participation conditions: ask questions and participate in live class discussions (if at any time I enroll in live courses), complete all personal study sections, assignments, quizzes and final exams in a timely manner, and strive to be on camera in the live classroom. Parent agrees to provide necessary technology and support student participation.
Agreement 2: Student and parent agrees with the honor code, which is that the student will strive to follow "For the Strength of Youth" at all times and in all places, and strive not to engage in bullying behavior.
Agreement 3: Student and parent agree that (if student enrolls in live courses) student name, voice, and image (e.g. video) may be recorded as part of the Ensign Peak Academy live classes, and that by enrolling in the program, I grant Ensign Peak Academy a full, perpetual, world-wide, royalty-free, and irrevocable license to share the live class recordings with other enrolled students. The purpose of the live class recordings is for students who have missed the live class or who wish to review the live class. Such recordings will not be shared in any other manner, with any other person or entity. I grant Ensign Peak Academy permission to publish screenshots of live classes in its yearbook. I also agree that, on other occasions, student may be asked to submit video assignments that may include display of student's name, voice, and/or video image. I grant Ensign Peak Academy a full, perpetual, world-wide, royalty-free, and irrevocable license to share these video assignments, including with fellow classmates. I agree that mentors may respond to video assignments with video and audio feedback. I acknowledge that Ensign Peak Academy will showcase examples of good student work on its blog or other online channels, and I grant permission for Ensign Peak Academy to showcase student submissions on its website, yearbook or other online channels. GRADE LEVELS You will be asked to indicate the student's grade level. The following grade level dates are provided as guidelines and are not requirements. If a grade level outside of these guidelines is preferred, please call or email the program director to discuss and request approval prior to submitting this application.
Middle School: 7th Grade = 12 years old by Sept 1, 2023 8th Grade = 13 years old by Sept 1, 2023
High School: 9th Grade = 14 years old by Sept 1, 2023 10th Grade = 15 years old by Sept 1, 2023 11th Grade = 16 years old by Sept 1, 2023 12th Grade = 17 years old by Sept 1, 2023 Note: Student and Parent email addresses MUST be different. |
