2022-2023 Middle and High School Application

We are delighted for the opportunity to share our programs with you. Our courses have been thoughtfully prepared to inspire and engage each individual student in a rich academic adventure, taught in light of the restored gospel.

Please complete the form below to apply for admission to Ensign Peak Academy's Middle School or High School Program, for live or self-paced courses.

A separate application is required for each new student. 
 

STUDENT AGREEMENTS
 

Please read the following agreements. Indicate agreement below.


Student Agreement 1: As a student of Ensign Peak Academy, I agree to the following participation conditions: I agree to ask questions and participate in live class discussions (if at any time I enroll in live courses), complete all personal study sections, assignments, quizzes and final exams in a timely manner, and strive to be on camera in the live classroom.


Student Agreement 2: I agree to the following student honor code agreement, which is to strive to follow "For the Strength of Youth" standards (e.g. language, music, media, etc.) at all times and in all places, and strive not to engage in bullying behavior.


Student Agreement 3: I agree that (if I enroll in live courses) my name, voice, and image (e.g. video) may be recorded as part of the Ensign Peak Academy live classes, and that by enrolling in the program, I grant Ensign Peak Academy a full, perpetual, world-wide, royalty-free, and irrevocable license to share the live class recordings with other students enrolled in Ensign Peak Academy. The purpose of the live class recordings is for students who have missed the live class or who wish to review the live class. Such recordings will not be shared in any other manner, with any other person or entity. I grant Ensign Peak Academy permission to publish screenshots of live classes in its yearbook. I also agree that, on other occasions, I may be asked to submit video assignments that may include display of my name, voice, and/or video image. I grant Ensign Peak Academy a full, perpetual, world-wide, royalty-free, and irrevocable license to share these video assignments, including with my fellow classmates. I agree that my mentors may respond to video assignments with video and audio feedback. I acknowledge that Ensign Peak Academy will showcase examples of good student work on its blog or other online channels, and I grant permission for Ensign Peak Academy to showcase my submissions on its website, yearbook or other online channels.

You will be asked to indicate your grade level. The following grade level dates are provided as guidelines and are not requirements. If a grade level outside of these guidelines is preferred, please call or email the program director to discuss and request approval prior to submitting this application.


Middle School:

7th Grade = 12 years old by Sept 1, 2022

8th Grade = 13 years old by Sept 1, 2022


High School:

9th Grade = 14 years old by Sept 1, 2022

10th Grade = 15 years old by Sept 1, 2022

11th Grade = 16 years old by Sept 1, 2022

12th Grade = 17 years old by Sept 1, 2022

Educational and Compulsory Attendance Laws
 
Ensign Peak Academy is a Utah-based accredited private online school established to serve homeschooling families. All parents and students fully assume the responsibility of ensuring that their participation in Ensign Peak Academy will satisfy the educational and compulsory attendance laws of the locality, state, and/or nation in which they reside, and they agree to hold Ensign Peak Academy harmless if such requirements are not satisfied. There is no guaranty, express or implied, that the coursework provided by Ensign Peak will satisfy the graduation requirements of any particular state, or the entrance requirements of any particular college or university. Additionally, Ensign Peak Academy works hard to ensure the success of our students by providing quality materials, instructors, and learning environment for our students. However, these tools do not guarantee the success of a student, and these tools may be improved over time. Ultimately, the success of the students depends upon the students themselves, the amount of parental involvement, and other factors that are not within the control of Ensign Peak Academy. By submitting this application, parents and students agree to commit their best efforts to learning and achieving success, and accept full responsibility for their success as Ensign Peak Academy cannot warrant or guaranty success beyond what the parents and students are willing to work to achieve.
