La catedral de Módena es la iglesia principal de la ciudad y de la archidiócesis de Módena-Nonantola. Una obra maestra del románico, fue construida por el arquitecto Lanfranco en el lugar de enterramiento de San Geminiano, patrón de Módena, donde anteriormente habían estado dos iglesias del siglo V. En la cripta de la catedral hay dos reliquias del santo, conservadas en una sencilla urna del siglo IV cubierta por una losa de piedra y sostenida por columnas expropiadas. El sarcófago, conservado en una vitrina, se abre cada año el día de la fiesta del santo, el 31 de enero. Los restos del santo, vestidos con el atuendo de obispo y con el conjunto pastoral a su lado, se exhiben para la devoción de los fieles. Al lado de la catedral se encuentra el campanario conocido como Ghirlandina. La catedral de Módena, la Ghirlandina y la Piazza Grande fueron declaradas Patrimonio de la Humanidad por la UNESCO en 1997. La catedral fue restaurada para reforzarla entre 2007 y 2008. Se sustituyeron algunas vigas de soporte que se habían deteriorado y se repararon las grietas de las paredes. También se restauró el rosetón, que se mostró bastante inestable, tanto sus pequeñas columnas (que se desmontaron y volvieron a montar una a una) como el cristal multicolor del siglo XV. El vidrio, en particular, se sometió a un análisis en profundidad llevado a cabo por la Universidad de Padua para identificar las partes no originales, que habían reemplazado las piezas originales a lo largo de los siglos (y después de las guerras) por vidrio de baja calidad; los estudios tenían como objetivo sustituirlas definitivamente por materiales más adecuados. En cuanto a la fachada, la intención es aprovechar la oportunidad para restaurar también los bajorrelieves de Wiligelmo que representan las Historias del Génesis (los que están a ambos lados de la entrada principal necesitan ser restaurados especialmente). El lado sur, el que da a Piazza Grande, también está programado para ser restaurado, ya que parece requerir algunos cuidados.
